How to create a Microsoft Form with live responses in Excel

Forms for Excel

From OneDrive for work

  • Sign in to Microsoft 365 with your NU credentials.
  • Select New à Forms for Excel to create a new form.

Excel for the web

From Microsoft Teams or SharePoint Online

  • Open the Excel workbook from the folder or channel in which you want to insert a form.
  • Select New à Excel workbook to open a New blank workbook.

  • Select Forms à New Form option from the ribbon (Insert tab) to create a survey, quiz, or poll.
